Requirements:
- Duly accomplished Online Passport Application Form https://www.passport.gov.ph ;
- Police Report;
- Notarized Affidavit of Loss (cite the passport number, Date and Place of Issue of the Lost Passport);
- Photocopy of the Lost Passport;
- Please note that there is no need to submit photos as that will be taken by machines during enrolment and data capturing process.
- Affidavit of Waiver (if there is no proof of visa/expired visa);
- PSA copy of Birth Certificate and Marriage Certificate (for women using married name);
- Additional Identification cards
