Requirements for First Time Applicant

    • Duly accomplished application form via Online Appointment https://www.passport.gov.ph

    • Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO.

    • Proof of identity / Supporting documents indicating full name, date and place of birth and citizenship

    • Other Supporting documents

In case of no record of birth: If born after 1950: 

      • Authenticated Late Registered Birth Certificate ; and Baptismal Certificate or Voter’s Affidavit or Certified True Copy of Voter’s Affidavit from the Commission on Election (COMELEC) or any public document indicating date and place of birth and citizenship.

If born on or before 1950:

      •  Certificate of Non-availability of Birth Record from the Local Civil Registrar’s Office;

      • Joint Birth Affidavit signed and executed by two (2) disinterested persons

      • Baptismal Certificate or Voter’s Affidavit or Certified True Copy Voter’s Affidavit from  COMELEC or any public

      • Please note that there is no need to submit photos  as that will be taken by machines during enrolment and data capturing process.

Personal appearance except when the applicant is:

      • Not more than 8 years old, or at least 65 years old, or mentally or physically incapacitated

      • Submit completed e-Passport Application Form

      • All passport applicants are required to Personally appear (Personal Appearance Only) at the Philippine Embassy.

Additional Requirements

1. For married women who chose to adopt surname of husband

For marriages performed in the Philippines, Certified True Copy and photocopy of Marriage Contract (MC) issued by the National Statistics Office (NSO) or CTC and photocopy of MC issued by the Local Civil Registrar duly authenticated by NSO, Malacanang Legal Office and Department of Foreign Affairs Authentication Unit

For marriages performed in Mexico, Certified True Copy, English Translation and photocopy of Marriage Contract (MC) issued by National Registration Department, and authenticated by the Ministry of Foreign Affairs, Putrajaya.

For marriages performed in other countries, original and Certified True Copy (photocopy) of Marriage Contract (MC) issued by the host government's registry of marriages and authenticated by that government's Embassy.

2. For women who obtained annulment or was divorced by foreign husband

Authenticated copy and photocopy of the first page and the dispositive portion of the judgment granting divorce or annulment; and,
Certified true copy and photocopy of the first page and the dispositive portion of the judgment granting divorce and annulment, authenticated by the Philippine Embassy/Consulate where the divorce was obtained.

3. For fiancés of foreign nationals and spouses of foreign nationals who were married at least six months prior to their application for a passport:

Original and photocopy of Commission of Filipino Overseas (CFC) Guidance and Counseling Certificate of Attendance.

4. For Minors (below 18 years old)

Personal appearance of either parent ; and if minor is not traveling with either parent, submit an original and photocopy of DSWD Clearance and Affidavit of Support and Consent.

i)   If both parents are abroad, such an affidavit must be authenticated by the nearest Embassy or Consulate General
ii)  If application is filed by person other than the minor’s parents, submit Special Power of Attorney (SPA). If executed abroad, SPA must be authenticated by the nearest Embassy or Consulate General ; and
iii) Passport and photocopy of the passport person traveling with the minor.

5. For Adopted Children

i)   Authenticated copy and photocopy of the dispositive portion of the Court Order on adoption;and
ii)  Authenticated copy and photocopy of original and amended Birth Certificate

6. For Government Officials and Employees

i)   For official passport, Travel Authority from the Head of Office/Department.
ii)  For regular passport, letter of no objection from the agency where employed.

7. For Muslim applicants

i)   For those whose births were not registered:

  • Original and Photocopy of Voter’s Affidavit or NBI Clearance or other supporting documents indicating date and place of birth and citizenship
  • Certificate from the Office of Muslim Affairs, Office of the President.

8. For Muslim Converts

    • Court order to change name and such other documentary requirements as may be deemed necessary by the passport offices or consular posts abroad Applicants with doubtful citizenship may be referred to the Citizenship Evaluation Committee (CEC) for interview and evaluation.
